Plain and simple form builder with plenty of analytics.
Key Points
1. User will be able to create a form and add any number of pages. Each page can be of type:
a. Info Page (Show an info to the user)
b. Yes or No Page (User responds with an yes or no)
c. MCQ Page (User chooses an option)
d. Text Entry Page (User enters a text)
e. Multiline Text Entry Page (User enters a long text)
f. Rating Page (User gives a rating)
2. A public URL will be shown when the form is published.
3. Users can answer the form using this public URL.
4. Responses to the form will be shown in a detailed manner on the webapp.
